What Is the NMPA? China's National Regulator for Medical Device Registration
The National Medical Products Administration (NMPA) is China's national authority responsible for the registration, supervision and standard-setting of drugs, medical devices and cosmetics. It operates under the State Administration for Market Regulation (SAMR) and was established in 2018 as the successor to the former China Food and Drug Administration (CFDA). Under the official “Provisions for Medical Device Registration and Filing” (SAMR Decree No. 47, in effect since 1 October 2021), the NMPA is nationally responsible for administering medical device registration and filing: it organizes the review and approval of domestic Class III devices and imported Class II and Class III devices, handles the filing of imported Class I devices, and supervises registration work carried out by provincial drug regulatory departments.
What Is the CMDE? China's Technical Review Body
The Center for Medical Device Evaluation (CMDE) is not a separate authority but a technical institution affiliated with the NMPA. According to the NMPA's official description of its affiliated institutions, the CMDE is responsible for accepting and conducting the technical review of registration applications for domestic Class III products and all imported medical devices, as well as for the filing of imported Class I products. It also contributes to drafting laws, regulations and technical guidance documents, develops review standards, evaluates emerging products such as regenerative medicine and tissue-engineered devices, and provides technical support to provincial review bodies.
In short: the NMPA makes the administrative decision based on CMDE review result and QMS inspection result — the registration itself — while the CMDE performs the underlying technical evaluation of the dossier. In practice, companies communicate directly with the CMDE during technical review, while the formal registration certificate is issued by the NMPA, or by the relevant provincial authority for domestic Class II devices.
Who Else Is Involved? The Role of Provincial Authorities
Not every product goes through the NMPA and CMDE. Under the official Provisions, the medical products regulatory deparment of provinces, autonomous regions and municipalities review the registration of domestic Class II devices and inspect the quality management systems of domestic Class II and Class III manufacturers. Filing of domestic Class I devices is handled at the city level. For imported products, the rule is simpler: Class I is filed with the NMPA, and Class II and III are both reviewed and approved by the NMPA — regardless of risk class, an imported device is always handled at the national level.
How China's System Differs Fundamentally From the EU
Under the EU Medical Device Regulation (MDR), manufacturers engage a private, EU-notified “Notified Body” to carry out conformity assessment of higher-risk devices. This model does not exist in China: technical review sits with a state institution — the CMDE or the relevant provincial authority — rather than with a third party. This has direct consequences for timelines, communication, and expectation management in any registration project.

ISO 13485 certification provides the structural foundation on which international registration dossiers — including for the Chinese market — can be built. Companies that already hold MDSAP certification, covering Australia, Brazil, Canada, Japan and the US in a single audit, bring valuable experience in working across multiple regulatory frameworks. It is worth noting, however, that MDSAP itself does not cover China, since China is not part of the program — a separate QMS inspection procedure China GMP is always required for domestic manufacturers, but for oversea manufacturers, the ISO 13485 certification can support the registration, NMPA doesn’t conduct pre-market QMS inspection for oversea manufacturers, instead NMPA may conduct the QMS inspection against China GMP after approval.

The Role of a Certified Quality Management System (ISO 13485) in China Registration
ISO 13485 certification is not, by itself, a formal legal prerequisite of the NMPA process. However, the official Provisions require applicants to establish a quality management system suitable for their products (Article 15) and to submit quality management system documentation related to product development and manufacturing as part of the registration dossier (Article 47); for domestic Class II and Class III devices, the relevant medical products regulatory departmentmay also inspect this system on-site (Articles 48–50). In practice, many manufacturers preparing a China dossier already maintain ISO 13485 certification for other markets, since it covers much of the same ground these inspections look at: design and development controls, risk management, and production and process controls. A well-maintained, independently certified quality management system does not replace the NMPA/CMDE technical review, but it gives manufacturers a documented starting point that is easier to adapt to China's specific requirements than building QMS evidence from scratch. The QMS requirements in China is the GMP for Medical devices. NMPA issues the new GMP for medical device at the end of 2025, the new GMP will come into force on 2026-11-1. In addition, imported medical devices are generally required to provide proof of marketing approval in their country or region of origin as part of the Chinese registration dossier. Since market approval in many jurisdictions is closely tied to compliance with a certified quality management system, often based on ISO 13485, manufacturers typically already hold ISO 13485 certification before entering the Chinese registration pathway.
